Incident Report

Please use the following Incident Report to inform the Title IX Coordinator about any Title IX-related incidents you have witnessed or been a victim of while at AMDA. This includes bullying, stalking, harassment, sexual assault, and other forms of sexual or gender based discrimination. Though you are not required to submit identifying information, the more specifics you include (contact information, descriptions, etc.), the more likely it will be that AMDA will be able to investigate the incident and mitigate the likelihood of a repeat incident.
